Client Consent, Liability Waiver & Testing Agreement

By signing this document, I acknowledge that I am voluntarily requesting services from DNA Confirmed for specimen collection and/or testing services, including but not limited to DNA testing, phlebotomy (blood collection), drug testing, and other laboratory specimen collection services.

1. Consent to Testing

I voluntarily consent to the collection of biological specimens including but not limited to:

• Blood

• Buccal (cheek) swabs

• Urine

• Saliva

• Hair or other approved samples

I understand these samples may be collected for the purpose of DNA analysis, drug screening, identity verification, or other laboratory testing purposes.

2. Authorization for Specimen Collection

I authorize trained personnel of DNA Confirmed to collect the required sample(s). I understand that specimen collection procedures may involve minor discomfort such as:

• Needle insertion for blood collection

• Skin puncture

• Mild bruising

• Temporary soreness

I acknowledge that these risks are minimal but possible.

3. Release of Liability

I agree to release, waive, and hold harmless DNA Confirmed, its owners, employees, contractors, affiliates, and laboratory partners from any and all liability, claims, damages, or injuries that may arise from:

• Specimen collection procedures

• Minor complications related to phlebotomy

• Laboratory testing processes

• Delays or processing times outside the company’s control

4. Accuracy of Information

I certify that the personal information I provide is true, accurate, and complete. I understand that providing incorrect identification or information may impact the validity of test results.

DNA Confirmed is not responsible for incorrect results caused by inaccurate information provided by the client.

5. Laboratory Processing

I understand that DNA Confirmed is a specimen collection service and may partner with certified third-party laboratories to process samples.

DNA Confirmed does not control laboratory analysis or result interpretation once the specimen has been transferred to the laboratory.

6. No Medical Diagnosis

I understand that testing performed through DNA Confirmed does not constitute medical advice, diagnosis, or treatment unless specifically provided by a licensed medical provider.

Clients are advised to consult a licensed physician or medical professional regarding any medical concerns.

7. Drug Testing Disclosure

For drug testing services, I understand that:

• Results may be used for employment, legal, or personal purposes

• Chain-of-custody procedures may apply

• Certain results may require confirmatory laboratory testing

DNA Confirmed is not responsible for employer decisions, legal outcomes, or disciplinary actions resulting from test results.

8. DNA Testing Acknowledgment

For DNA testing services, I acknowledge that:

• Results reflect genetic probability based on submitted samples

• DNA Confirmed is not responsible for emotional, legal, or family outcomes resulting from test results

• Certain tests may not be legally admissible unless ordered under a legal chain-of-custody process

9. Privacy and Confidentiality

DNA Confirmed respects the privacy of all clients. Personal information and test results will be handled in accordance with applicable privacy and health information laws.

However, results may be released:

• To the client

• To authorized parties listed on the consent form

• As required by law

10. Payment and Refund Policy

All services must be paid in full prior to specimen collection unless otherwise stated.

Due to the nature of laboratory processing:

• All sales are final

• No refunds are issued once a specimen has been collected or submitted for testing.

11. Client Responsibility

I agree to follow all instructions provided by DNA Confirmed staff during the specimen collection process.

Failure to follow instructions may result in invalid or rejected samples, and additional fees may apply for recollection.

12. Acknowledgment

By signing below, I confirm that:

• I have read and understand this consent and liability waiver

• I voluntarily agree to testing services

• I release DNA Confirmed from liability as described above
